PROCESS TO MAKE MARRIAGE CERTIFICATE


FORM NEEDED- FORM D- MEMORANDUM OF MARRIAGE

·         This form is valid only for marriages done in Maharashtra.

 

DOCUMENTS NEEDED:

  • Bride & Bridegroom Age Proof: Leaving Certificate OR Passport OR Birth Certificate R Domicile Certificate OR PAN Card
  • Bride & Bridegroom Residential Proof : Ration Card OR Passport OR Election Card OR Light Bill And Telephone Bill Of Own Name.
  • Three (3)Witness Proof :- Ration Card OR Passport OR Election Card OR Driving License OR Govt. Identity Card.
  • Photographs of the 3 witness.
  • Signature and details of the priest or pandit who did the wedding.
  • Wedding Card of both the bride and the bridegroom.
  • In Case Of Non Availability Of Wedding Card Rs 100=00 Stamp Paper Affidavit Of Both Husband Wife And Three Witnesses Is Required
  • Photographs Of Both Bridegroom / Bride


FEES OF APPLICATION:

·         If applied for registration within 90 days from the date of marriage- Rs.50/-

·         If applied for registration within 90 days to 1year from the date of marriage- Rs.150/-

·         If applied for registration after 1 year from the date of marriage- Rs.250/-


NOTE:

·         In case of widow or divorce, the death certificate or the divorce papers need to be submitted as the case may be.

·         All documents need to be presented in original along with one attested photocopy of each by a gazette officer.

·         In case of inter-caste marriages, the caste conversion of the bride needs to be shown on an affidavit with Rs.100/- stamp paper. Without this the marriage cannot be registered via the above mentioned form. This affidavit should also contain the name of the priest who has done the conversion.

·         Monday to Friday – 11:00am to 1pm


VENUE:

·         P- south ward office, Opposite City Centre, S.V Road, Goregaon(west), Mumbai.

·         You need to go to the health department for processing.
