PROCESS TO MAKE BIRTH CERTIFICATE

FORM NEEDED

• Birth certificate application form ( available at the centre)

DOCUMENTS NEEDED: 

·        You need to submit the discharge papers of the hospital where the birth took place.

·         Residential and identity proof of the parents may be demanded. So I suggest you better carry them in original with 1 attested photocopy.

FEES OF APPLICATION:
·         Fees vary according to the number of copies demanded.

·         Approximate cost is Rs. 20/- per copy

NOTE:
·         In case you already have a birth certificate but you require more copies, then the same process can be followed. At this time you don’t require the hospital discharge papers. You would need a photocopy of your original birth certificate alongwith a letter requesting to provide you with more copies.

TIME:
·         Monday to Friday – 9am to 1pm (But prefer to go before 12 noon to get it done the same say
